89B . Leaving care plan
A leaving care plan
for a child is a plan that —
(a)
identifies the needs of the child in preparing to leave the CEO’s care
and in transitioning to other living arrangements; and
(b)
outlines steps or measures to be taken to assist the child to meet those needs
(including the social services proposed to be provided when the child leaves
the CEO’s care).
