Western Australian Consolidated Regulations Western Australian Consolidated Regulations (1) Except in respect
of a day on which a member becomes incapacitated while on duty, an application
for leave by a member on account of incapacity shall be supported by a
certificate of a medical practitioner or, where the incapacity involves a
dental condition, by a certificate of a dentist.
(2) The application
shall be —
(a) in a
form approved by the Commissioner; and
(b)
submitted to the Manager,
and the certificate in
its support shall be —
(c)
submitted to the Manager.
