registration as the provider of a taxi dispatch service shall be in the
approved form and shall —
(a) be
made by the person intending to provide the taxi dispatch service;
(b)
where the application is made by a partnership, disclose the name and address
of each member of the partnership;
(c)
where the application is made by a corporation, either solely or as a member
of a partnership, disclose the name and address of the directors, the other
persons concerned in the management of the corporation, and the shareholders
of the corporation; and
(d)
disclose the identity of any other person who is to have a financial interest
in the taxi dispatch service.
