Western Australian Consolidated Acts Western Australian Consolidated Acts (1) An application for
an adoption order is to be in the form prescribed by rule.
(2) An application for
an adoption order in relation to a child is to be filed in the Court together
with a written memorandum of the provisions of the adoption plan in relation
to the child that is signed by or on behalf of the parties to the plan.
(3) The requirement in
subsection (2) to file a memorandum of the provisions of the adoption
plan does not apply where an application will be made for the Court to
dispense with the requirement for an adoption plan.
