16. Application for authorisation under section 28A

A person applying under section 28A(4)(a) of the Act for an authorisation
must-

   (a)  complete the appropriate application form provided by the Secretary in
        respect of the purpose for which the authorisation is required; and

   (b)  submit the form to the Secretary.
