222. Form of document

A driver licence or permit document must show details of the following-

   (a)  an identification number for the person to whom it is issued;

   (b)  the first name, second and third initials (if any), and family name of
        the person;

   (c)  a photograph or digitised image of the person;

   (d)  the person's residential address;

   (e)  the person's date of birth;

   (f)  the person's signature (or a reproduction of that signature);

   (g)  the category or categories of driver licence or permit held by the
        person in accordance with regulation 202;

   (h)  the expiry date of the licence or permit;

        (i)    the code of any condition to which the licence or permit is
               subject in accordance with regulation 207.
