Certificate under section 57A(3)

212. Certificate under section 57A(3)

A certificate under section 57A(3) of the Act must contain the following
particulars-

   (a)  a statement by the registered medical practitioner or approved health
        professional as to whether the requirements of the Act and these
        Regulations for the collection of urine samples have been complied
        with;

   (b)  the name of the person from whom the urine sample was collected or, if
        the name of the person is not known, sufficient information to enable
        the sample to be identified with the person from whom it was
        collected;

   (c)  the time and date the urine sample was collected;

   (d)  the name and signature of the registered medical practitioner or
        approved health professional who collected the urine sample.
