14. Register kept by approved doctor

For the purposes of section 63(2) and (3) of the Act-

   (a)  the prescribed manner for recording information in the Register is-

   (i)  to print or write the information in a legible form in the English
        language in a book or loose-leaf binder; or

   (ii) in an electronic form that is readily convertible into legible print
        in the English language;

   (b)  the prescribed information required to be recorded in the Register is
        the information listed in Schedule 2.
