Particulars of demand

21. Particulars of demand

In every such action the plaintiff on the record shall be required together
with the statement of claim to deliver to the defendant or his legal
practitioner full particulars-

   (a)  of the person or persons for whom and on whose behalf such action is
        brought;

   (b)  of the nature of the claim in respect of which damages are sought to
        be recovered;

   (c)  of the date of birth of each such person and of the deceased person;

   (d)  of the name and address of each person by whom the deceased person was
        employed at any time within the period of twelve months preceding the
        day on which he suffered the injury which caused his death;

   (e)  of the date of commencement of each such employment and the duration
        thereof;

   (f)  of the total net sum (after deducting instalments of tax deductible
        therefrom) that the deceased earned in the period of twelve months
        referred to in paragraph (d).
