Approved form to be used

36. Approved form to be used



(1) If the Authority has approved a form for the giving of consent, a person
giving consent under Part 2 or 3 must give it on the approved form.

(2) The Authority may approve a different form according to place, class of
person, type of procedure or other circumstance.
