51. Application on oath, by affidavit or certified if warrant to issue

If the applicant for a family violence intervention order seeks a warrant to
issue in the first instance for the arrest of the respondent-

   (a)  the application for the warrant must be in writing and be made on oath
        or by affidavit or be certified; and

   (b)  the application for the family violence intervention order and the
        warrant must be in the same document and contain the prescribed
        particulars.
