20. Further information about family day care service



(1) An application under section 16 must include-

   (a)  the address that is to be the service's principal office; and

   (b)  the address of any family day care venue proposed to be used by the
        service; and

   (c)  a statement of the process proposed to be used by the applicant to
        approve-

   (i)  a family day carer; and

   (ii) a family day carer's residence.
