19. Further information about approval of premises

An application under section 15 must also be accompanied by-

   (a)  a copy of the current certificate of approval of premises; and

   (b)  details of any circumstances that have changed with respect to the
        design or location of the premises where the service is to operate
        since the approval was granted.
