CHILDREN'S SERVICES ACT 1996 - SECT 11

An application under section 10 must—

        (a)     be made to the Regulatory Authority; and

        (b)     be in writing; and

        (c)     include the prescribed information; and

        (d)     include payment of the prescribed fee.
