2. | Fee payable to a medical practitioner for the carrying out of a medical examination under section 9 of the Main Act (including the taking of any samples of blood or urine in connection with that examination) | 30.00 |
3. | Fee payable to a qualified nurse for the taking of a sample of blood | 30.00 |
