If a council grants an application for a licence to carry out any public health risk activity, it is to –

(a) issue the licence in an approved form; and

(b) endorse the licence with any conditions; and

(c) specify on the licence –

(i) the name of the person to whom it is issued; and

(ii) the date of issue of the licence.
