ANTI-DISCRIMINATION ACT 1998 - SECT 62
Form of complaint
(1) A complaint is to (a) be made in writing and signed by the complainant; and(b) identify the person, class of persons or organisation against whom the alleged discrimination or prohibited conduct was directed and against whom the complaint is made; and(c) set out details of the alleged discrimination or prohibited conduct; and(d) be lodged with the Commissioner (i) in person; or(ii) by post; or(iii) by any other means the Commissioner allows.(2) The Commissioner may provide procedural advice and assistance to any person who requires assistance to make a complaint.
