South Australian Consolidated RegulationsAn application under section 32(1) of the Act by a party to a dispute between
the administering authority of a retirement village and a resident of the
retirement village to the Tribunal for resolution of matters in dispute—
(a) must
include the following information:
(i)
the name and address of the administering authority;
(ii)
the name and address of the retirement village;
(iii)
the name of the resident;
(iv)
the address for service of notices on the resident;
(v)
the reasons for the application;
(vi)
the relief sought from the Tribunal;
(vii)
whether the other party to the dispute has been advised
of the application; and
(b) must
be signed and dated by the applicant; and
(c) must
be accompanied by—
(i)
a copy of the residence contract; and
(ii)
a copy of the residence rules; and
(iii)
the prescribed fee.
