SURROGACY ACT 2010 No. 2 - SECT 34

A parentage order must state the following details--

(a) the date of the order;

(b) the first name and surname of the child--

(i) before the order was made; and

(ii) on the making of the order;

(c) the date of birth of the child;

(d) the place of birth of the child;

(e) for each applicant, the applicant's first name and surname, address and occupation;

(f) the first name and surname and address of each of the the child's birth parents;

(g) the terms of any ancillary orders.
