Queensland Consolidated Regulations Queensland Consolidated Regulations(1) A registered service provider may surrender the provider's registration by written notice given to the chief executive.
(2) The surrender takes effect--
(a) on the day the notice is given; or
(b) if a later day is stated in the notice, on the later day.
(3) The provider must, before the surrender takes effect, ensure that any competency declaration forms issued by the chief executive to the provider that are unused are returned to the chief executive.
Maximum penalty--40 penalty units.
