Queensland Consolidated Regulations(1) An accredited person may apply to the chief executive for the replacement of an accreditation document or identity card if the document or card is lost, damaged, destroyed or stolen.
(2) The application must--
(a) be made in the approved form; and
(b) be accompanied by the appropriate fee.
(3) If the chief executive is satisfied the accreditation document or identity card has been lost, damaged, destroyed or stolen, the chief executive must replace it.
