Queensland Consolidated Acts Queensland Consolidated Acts(1) A person who is eligible to apply for a certificate may apply to the Authority for a certificate.
(2) The application must--
(a) be in the approved form; and
(b) be accompanied by the application fee and the certificate fee prescribed under a regulation; and
(c) state the following information--
(i) the applicant's name, address, occupation and date of birth;
(ii) the names, addresses and occupations of all associates of the applicant and, if known to the applicant, their dates of birth;
(iii) anything else prescribed under a regulation.
