(1) A person may, by notice (a
"service request" ), ask the agency to fund particular treatment, care or
support (the
"requested service" ) to be provided to a participant in a particular period.
(3) If a service request is made by a person who is not the
participant, the person making the request must give a copy of the request to
the participant.
