LOTTERIES ACT 1997 - SECT 52

(1) A key person licence must be in the approved form.

(2) The approved form must provide for the inclusion of the following particulars--

(a) the key person licensee's name;

(b) a recent photograph of the licensee;

(c) the date of issue of the licence;

(d) whether the licensee is a licensed employee or licensed as a key operator;

(e) the conditions of the licence;

(f) other particulars prescribed under a regulation.
