125 Application for, or renewal of, licence
(1) An application for, or renewal of, a licence to provide care services
must—
(a) be made by a corporation; and
(b) be made to the chief executive
in the approved form; and
(c) nominate an adult to be nominee for the
licence; and
(d) be accompanied by a notice that—
(i) is signed by the
nominee for the licence; and
(ii) states whether or not, for each person
mentioned in section 126(b) (i) or (ii) , the person holds a working with
children authority or negative notice; and
(iii) if the person holds a
working with children authority—states the expiry date for the authority;
and
(iv) if the person does not hold a working with children
authority—states whether or not the person has made a working with children
check application and whether the application has been decided or withdrawn;
and
(e) if the application is for renewal of a licence—be made at least 30
days before the licence ends.
(2) The application form approved by the chief
executive may require the disclosure of the domestic violence history and
traffic history of a person to whom section 142A(a) applies.
