New South Wales Consolidated Regulations New South Wales Consolidated RegulationsFor the purposes of section 288 (b) of the Act, the prescribed particulars to be specified when giving notice of having or ceasing to have a relevant interest in the right to vote of a member of a co-operative are as follows:
(a) the name of the co-operative to whom notice is being given,
(b) the full name and address of the person giving notice,
(c) the date on which the relevant interest was acquired or ceased,
(d) the name of the member whose right to vote was so affected,
(e) the date of giving notice.
