CO-OPERATIVES REGULATION 2005 - REG 16

16 Notice of appointment etc of directors and officers

For the purposes of section 251B (2) (c) of the Act, the following particulars are prescribed:

(a) the name of the co-operative or subsidiary,

(b) the name and position of the person giving notice of the appointment or cessation of appointment,

(c) in respect of any person being appointed to act as a director, principal executive officer or secretary:

(i) the full name (family and given names), and

(ii) any former names, and

(iii) the residential address, and

(iv) the date of birth, and

(v) the office held and date appointed,

(d) in respect of any person ceasing to hold that office:

(i) the full name (family and given names), and

(ii) the date of birth, and

(iii) the office held and date appointment ceased,

(e) in respect of any change of name of a director, principal executive officer or secretary:

(i) the name previously notified, and

(ii) the new name (family and given names), and

(iii) the date of change, and

(iv) the date of birth, and

(v) the office held,

(f) in respect of any change of address of a director, principal executive officer or secretary:

(i) the new address, and

(ii) the date of change, and

(iii) the date of birth, and

(iv) the office held.
