"RCA team" means a root cause analysis team appointed under section 20M.
"relevant health services organisation" means:
(a) an area health service, or
(b) a statutory health corporation prescribed by the regulations, or
(c) an affiliated health organisation prescribed by the regulations, or
(d) in the case of an incident involving the provision of ambulance services under Chapter 5A of the Health Services Act 1997 or the provision of health support services under Part 1A of Chapter 10 of that Act-the Director-General."reportable incident" means an incident relating to the provision of health services by a relevant health services organisation, being an incident of a type prescribed by the regulations or set out in a document adopted by the regulations.
