(b) include or be accompanied by information
or evidence required by the Secretary to assess the application.
(3) The
regulations may provide for a fee to be paid for an application for the
cancellation or suspension of registration.
(4) The Secretary may require the
fee to accompany the application.
(5) A reference in this section to an
application for suspension of registration is taken to include a reference to
an application to extend, cancel or vary a suspension imposed under this
section.
