(1) The Secretary must make a written report to the Minister every year on the
reportable deaths known to the Secretary that occurred during the previous
calendar year.
(a) who was
the subject of a risk of significant harm report made during the period of 3
years immediately preceding the death of the child, or
(b) who was a sibling
of a child or young person who was the subject of a risk of significant harm
report made during the period of 3 years immediately preceding the death of
the sibling, or
"risk of significant harm report" means a report given to the Secretary under
this Act that, in the assessment of the Secretary, indicates that a child or
young person is at risk of significant harm (within the meaning of Part 2 of
Chapter 3).
(3) A report under this section must include the
following--
(a) the number of reportable deaths that occurred during the
calendar year to which the report relates,
(b) the general circumstances of
each of those reportable deaths, if known,
(c) details relating to the
implementation of any Departmental practice changes in response to, or
resulting from, those reportable deaths.
(4) A report under this section is
to be made to the Minister as soon as practicable after the end of the
calendar year to which the report relates.
(5) The Minister is to cause a
copy of the report made to the Minister under this section to be tabled in
each House of Parliament as soon as practicable after the report is made to
the Minister.
