Certification of copy of report 
5.3.02. A copy of:

   (a)  a report lodged under subsection 456 (12), (13) or (14) of the
        Corporations Law; and

   (b)  a certificate or other document annexed to that report; must be
        certified in writing by the person who prepared it to be a true copy.
