AUSTRALIAN INDUSTRIAL RELATIONS COMMISSION RULES 1998 No. 1 - REG 5

(1) Subject to subrule (3), a form in the Schedule must be used if it is
applicable.

(2) An application for which no form is provided in the Regulations or these
Rules must:

   (a)  set out the name of the applicant; and

   (b)  set out the nature of the matter and the grounds on which the
        application is made; and

   (c)  be lodged in the Industrial Registry.

(3) It is sufficient compliance with these Rules as to the form of a document
if the document is substantially in accordance with the required form of the
document or has only such variations as the nature of the case requires.
