CHILDCARE REBATE ACT 1993 No. 112 of 1993 - SECT 33

33.(1) The claim must specify:

   (a)  the name of each dependent child who was provided with the child care
        to which the claim relates; and

   (b)  the Family Registration Number of the family to which the claimant and
        the child or children belong; and

   (c)  the Child Care Provider Number of each registered carer who provided
        the child care; and

   (d)  the period during which the child care was provided; and

   (e)  the amounts paid to registered carers for the child care; and

   (f)  such other information as is specified in the approved form for the
        claim.

(2) The claim must be accompanied by such other documents as are specified in
the approved form for the claim.

(3) The amounts specified in the claim as amounts paid to registered carers
for the child care must not include any amounts of fee relief that have not
yet been deducted from the amounts so paid.
