Commonwealth Consolidated Regulations (1) A person must file
with an application mentioned in an item of Table 4.1, the documents mentioned
in the item.

Note The documents required to be filed with an application
under this rule are in addition to the documents required to be filed under
rule 2.02.
(2) For paragraph (c)
of item 1 of Table 4.1, if the applicant does not have a copy of a document
lodged by the other party with the Child Support Agency, the applicant may
file the summary of the document prepared by the Child Support Agency.
