PUBLIC HEALTH ACT 1997 - SECT 46

Procedure licence—form

A procedure licence must state—

        (a)     the name of the licensee; and

        (b)     the licensed public health risk procedure; and

        (c)     any conditions to which the licence is subject.

Note     If a form is approved under s 137A (Approved forms) for a licence, the form must be used.
