PUBLIC HEALTH ACT 1997 - SECT 31

An activity licence must state the following:

        (a)     the name of the licensee;

        (b)     the licensed public health risk activity;

        (c)     the licensed premises;

        (d)     the term for which the licence is granted;

        (e)     any conditions to which the licence is subject.

Note     If a form is approved under s 137A (Approved forms) for a licence, the form must be used.
